payment by credit card

Здравствуйте.

У кого-нибудь есть заполненный payment by credit card?
Не знаю, нужно ли заполнять такие поля как:
1) amount
2) job offer (temporary или permanent), если у меня никакого нет
3) Undertaking — Family class
Undertaking — Humanitarian sponsorship
4) Comparative evaluation for studies done outside Quebec
